Abstract:
A care gap module operating on a central computing entity receives a trigger indication comprising patient identifying information; extracts the patient identifying information from the trigger indication; queries a plurality of care standards stored in a memory media to identify care standards relevant to the patient based on at least one of (a) demographic information corresponding to the patient or (b) medical history information corresponding to the patient; and analyzes the medical history information corresponding to the patient based on the care standards to identify whether the medical history information is missing any data items indicative of gaps in care. The central computing entity generates a care gap notification that indicates whether any gaps in care were identified and, when a gap in care was identified, indicates what the gap was; and provides the care gap notification such that a user computing entity receives the care gap notification.
